After Mordecai and Rigby read The Hungry, Hungry Soul-Sucking Death Worm to Thomas, they pull a tab which releases the actual Soul Worm. Thomas tried to feed Muscle Man's soul to it but Mordecai and Rigby managed to lure it back into the book.
