The fecal immunochemical test (FIT) is a diagnostic technique that examines stool samples for traces of non-visible blood, which could potentially indicate conditions including bowel cancer. [1] Symptoms which could be caused by bowel cancer and suggest a FIT include a change in bowel habit, anaemia , unexplained weight loss, and abdominal pain .
Fecal occult blood testing (FOBT), as its name implies, aims to detect subtle blood loss in the gastrointestinal tract, anywhere from the mouth to the colon.Positive tests ("positive stool") may result from either upper gastrointestinal bleeding or lower gastrointestinal bleeding and warrant further investigation for peptic ulcers or a malignancy (such as colorectal cancer or gastric cancer).
The Food and Drug Administration (FDA) has recently approved a blood test screening for colorectal cancer that has an over 83% success rate at detecting the presence of this form of cancer.
M2-PK, as measured in feces, is a potential tumor marker for colorectal cancer. When measured in feces with a cutoff value of 4 U/ml, its sensitivity has been estimated to be 85% (with a 95% confidence interval of 65 to 96%) for colon cancer and 56% (confidence interval 41–74%) for rectal cancer. The stool guaiac test method may be preferable to fecal immunochemical testing (FIT) if there is a clinical concern about possible gastric or proximal upper intestinal bleeding. [37] However, although heme breakdown is less than globin during intestinal transit, false negative results can be seen with the stool guaiac tests due to degradation ...
A multi-target stool DNA test was approved in August 2014 by the FDA as a screening test for non-symptomatic, average-risk adults 50 years or older. [8] A 2017 study found this testing to be less cost effective compared to colonoscopy or fecal occult blood testing. [ 9 ]
However, a number of diseases of the pancreas and gastrointestinal tract are characterized by fat malabsorption. Examples of such diseases are: disorders of exocrine pancreatic function, such as chronic pancreatitis, cystic fibrosis and Shwachman–Diamond syndrome (these are characterized by deficiency of pancreatic digestive enzymes)
[19] [20] [21] A single office-based FOBT (fecal occult blood test) performed following a digital rectal examination (DRE) is not an adequate screen due to low sensitivity for advanced tumor and colorectal cancer. [22] Screening for colon cancer this way does not meet HEDIS, Medicare or American Cancer Society standards. [23]
