For individual applicants, there are two ways to download the latest China Visa Application Form: from the official website of the Chinese Embassy and Consulates (in PDF/DOC), and from the recognized site of China Visa Application Service Center after you complete the form online.

Complete the online application form at the China Online Visa Application (COVA) website. Please refer to the consular jurisdiction of the Chinese Embassy and Consulate General in the United States and select corresponding Embassy/Consulate-General when you submit the form online.
Visa applicants (excluding those applying for Hong Kong SAR and Macau SAR visas) can access and fill out the E-application forms at http://cova.mfa.gov.cn, and print out the confirmation page and full application form (8 pages in total).
Applicants will spend much less time filling in the forms and expect a more streamlined visa application process. The new version of the visa application forms is launched on September 20, 2023. Both pre-optimized and post-optimized visa application forms can be submitted.
