Diagnosis of Wernicke-Korsakoff syndrome is by clinical impression and can sometimes be confirmed by a formal neuropsychological assessment. Wernicke encephalopathy typically presents with ataxia and nystagmus, and Korsakoff's psychosis with anterograde and retrograde amnesia and confabulation upon relevant lines of questioning. [25]
Korsakoff syndrome (KS) [1] is a disorder of the central nervous system characterized by amnesia, deficits in explicit memory, and confabulation. This neurological disorder is caused by a deficiency of thiamine (vitamin B 1 ) in the brain, and it is typically associated with and exacerbated by the prolonged, excessive ingestion of alcohol . [ 2 ]
When it occurs simultaneously with alcoholic Korsakoff syndrome it is known as Wernicke–Korsakoff syndrome. [3] [4] Classically, Wernicke encephalopathy is characterised by a triad of symptoms: ophthalmoplegia, ataxia, and confusion. Around 10% of patients exhibit all three features, and other symptoms may also be present. [5]
Wernicke encephalopathy: an acute neurological syndrome of ophthalmoparesis, ataxia, and encephalopathy brought on by thiamine deficiency. Wernicke–Korsakoff syndrome , also called Korsakoff psychosis : a subacute dementia syndrome, often following Wernicke encephalopathy, characterized clinically by confabulation and clinicopathologically ...
Korsakoff's syndrome and Wernicke–Korsakoff syndrome are particular forms of alcohol related brain injury which may be related to alcohol related dementia. [18] Many experts use the terms alcohol (or alcoholic) dementia to describe a specific form of ARD, characterized by impaired executive function (planning, thinking, and judgment).
Wernicke–Korsakoff syndrome can cause brain damage and results from a Vitamin B deficiency (specifically vitamin B1, thiamine). [29] [30] This syndrome presents with two conditions, Wernicke's encephalopathy and Korsakoff psychosis. Typically Wernicke's encephalopathy precedes symptoms of Korsakoff psychosis.
Treatment for confabulation is somewhat dependent on the cause or source, if identifiable. For example, treatment of Wernicke–Korsakoff syndrome involves large doses of vitamin B in order to reverse the thiamine deficiency. [62] If there is no known physiological cause, more general cognitive techniques may be used to treat confabulation.
Sergei Sergeyevich Korsakov (Russian: Серге́й Серге́евич Ко́рсаков; 22 January 1854, Gus-Khrustalny – 1 May 1900, Moscow) was a neuropsychiatrist from the Russian Empire, known for his studies on alcoholic psychosis. His name is lent to the eponymous Korsakov's syndrome and Wernicke–Korsakoff syndrome.
