Therefore in my case with JAWS 5.1, I get the image description read three times - the first and third time with a link to the image description page and the second time with a link to the page that is linked in the template. I can find the link just fine but the caption shouldn't be displayed three times for those using non-CSS browsers.
Clickable images are very cute, but Template:Click has too many problems (don't you think accessibility is more important than a pretty portal?). Ask Jimbo to enable that special syntax if you really want clickable images (maybe there it can be enabled in User: or Portal: namespaces without disrupting the articles).
