Posterior tibial tendon dysfunction is the dysfunction of the posterior tibial tendon. It is a progressive disease that has four stages [ 1 ] and is the most common cause of adult flatfoot . [ 2 ]
The tendon also can become inflamed, causing a painful condition called posterior tibial tendonitis or posterior tibial tendon dysfunction, according to the Cleveland Clinic.
The tibialis posterior has a major role in supporting the medial arch of the foot. Dysfunction of the tibialis posterior, including rupture of the tibialis posterior tendon, can lead to flat feet in adults, as well as a valgus deformity due to unopposed eversion when inversion is lost. [4] [5]
If non-invasive treatment measures fail, tarsal tunnel release surgery may be recommended. Tarsal tunnel release is a form of a nerve decompression to relieve pressure on the tibial nerve. The incision is made behind the ankle bone and then down towards but not as far as the bottom of foot. The posterior tibial nerve is identified above the ankle.
Portions of the posterior tibialis tendon sometimes insert onto the accessory ossicle, which can cause dysfunction, and therefore, symptoms. Type 3: A cornuate navicular bone represents an enlarged navicular tuberosity, which may represent a fused Type 2 accessory bone. Occasionally symptomatic due to bunion formation.
In the case of posterior tibial tendon dysfunction causing flatfoot, sinus tarsi syndrome can also develop due to the disruption in the entire structure of the foot. [4] The condition is also thought to be caused by ankle/subtalar joint instability causing inflammation in the area. [7]
The popliteus tendon's main attachment is on the femur at the proximal portion of the popliteus sulcus. As the tendon runs posteriorly and distally behind the knee, it gives off 3 fascicles that attach to and stabilize the lateral meniscus. The popliteus tendon provides static and dynamic stabilization to the knee during posterolateral rotation.
