Walter Jackson Freeman II (November 14, 1895 – May 31, 1972) was an American physician who specialized in lobotomy. [1] Wanting to simplify lobotomies so that it could be carried out by psychiatrists in psychiatric hospitals, where there were often no operating rooms, surgeons, or anesthesia and limited budgets, Freeman invented a transorbital lobotomy procedure.
It was invented by Dr. Walter Freeman in 1948 to replace the unique form of leucotome used up until that point for the transorbital lobotomy procedure. This instrument is, essentially, an ice pick with some gradation marks etched onto the shaft.
This new procedure became known as the transorbital lobotomy, also dubbed the "ice pick lobotomy" because the instrument used, an orbitoclast, was very similar to a common ice pick. The new procedure also signaled the end of the professional relationship between Freeman and Watts.
He hammered an ice pick-like instrument, an orbitoclast, through the eye socket and swept through the frontal lobes. The transorbital or "ice pick" lobotomy was done under local anesthesia or using electroconvulsive therapy to render the patient unconscious and could be performed in mental hospitals lacking surgical facilities. [1] In 1960, at 12 years of age, Dully was submitted by his father and stepmother for a trans-orbital lobotomy, performed by Freeman for $200 (equivalent to $2,060 in 2023). [2] During the procedure, a long, sharp instrument called an orbitoclast was inserted through each of Dully's eye sockets 7 centimeters (2.8 in) into his brain.
This season of “The Boys” continues to outdo itself in gross-out moments, and episode 4 has plenty of them — like an ice-pick lobotomy, a lasered-off penis and flame-broiled scientist. More ...

In 1946 Freeman developed the transorbital lobotomy, based on a technique first reported by Italian psychiatrist Amarro Fiamberti. [4] In this operation an ice-pick like instrument was inserted through the roof of the orbit (eye socket), driven in with a mallet, and swung to and fro to cut through the white matter.
