The site is apparently geologically unique in the Hawaiian Islands, comprising a sinkhole paleolake in a cave formed in eolianite limestone. The paleolake contains nearly 10,000 years of sedimentary record; since the discovery of Makauwahi as a fossil site, excavations have found pollen, seeds, diatoms, invertebrate shells, and Polynesian artifacts, as well as thousands of bird and fish bones.

The site consists at its core of a rock-hewn ancient burial cave with multiple burials. [6] Some 500 metres west of the cave is a Byzantine cemetery. [6] The cave is partly natural, partly man-made. The cave formation was the result of tectonic uplift of the karst at 2 to 5 million years ago. The caves are typical of the tropical river caves, which exhibits various features such as: elliptical tubes linking different levels of the cave, and speleothem including aragonite and calcite needles. The limestone are either extremely white or grey.
