Small defects of the upper and lower lip can be closed primarily. For the upper lip, defects of up to 1/4 (25%) of the lip may be closed primarily. For the lower lip, defects of up to 1/3 of the lip may be closed primarily. This means the edges of the defect are simply sutured together in three layers: oral mucosa, muscle, and skin.
The commissure is the corner of the mouth, where the vermillion border of the superior labium (upper lip) meets that of the inferior labium (lower lip). The commissure is important in facial appearance, particularly during some functions, including smiling. As such it is of interest to dental surgeons.
The vermilion border (sometimes spelled vermillion border), also called margin or zone, is the normally sharp demarcation between the lip and the adjacent normal skin. Timing is important to wound healing. Critically, the timing of wound re-epithelialization can decide the outcome of the healing. [11] If the epithelization of tissue over a denuded area is slow, a scar will form over many weeks, or months; [12] [13] If the epithelization of a wounded area is fast, the healing will result in regeneration.
The initial piercing is done as an incision of the lower lip of 1 to 2 cm length, and a simple wooden peg is inserted. After the wound has healed, which usually takes between two and three weeks, the peg is replaced with a slightly bigger one. At a diameter of about 4 cm, the first lip plate made of clay is inserted.
The development of signs and symptoms that clinically indicate a Morel-Lavallée lesion can occur quickly or slowly. The clinical presentation time frame ranges from hours to days after an injury and in some cases months to years. [7] [8] Visually, the area of the injury may have obvious signs or appear normal externally despite underlying damage.
In a major injury, if epithelial cell migration and tissue contraction cannot cover the wound, suturing the edges of the injured skin together, or even replacement of lost skin with skin grafts, may be required to restore the skin. As epithelial cells continue to migrate around the scab, the dermis is repaired by the activity of stem cells.
