The road runs from South Carolina Highway 177 (SC 177) at the North Carolina- South Carolina border, through the town of Hamlet to U.S. Highway 1 (US 1) just south of Hoffman and just north of Rockingham .
NC 381 is a two-lane rural highway that traverses 10.6 miles (17.1 km) from the South Carolina state line in Gibson to U.S. Route 74 Business near Hamlet. The highway is flanked with farms and has little traffic.
The S Line north of Hamlet was originally built in 1877 as part of the Raleigh and Augusta Air-Line Railroad. From Hamlet south, the line was originally part of the Palmetto Railroad and began service in 1887. [3] Both lines were merged into the Seaboard Air Line Railroad in 1901 and made up their main line.

It was founded by Johannes Cornelis Goosen, who was born in the Klein Drakenstein and came to the Warm Bokkeveld as a young farmer. In March 1851 Goosen bought the farm Wagenbooms Rivier from George Sebastiaan Wolfaardt at the “fantastic” price of £6 000.
