I have a sick fascination with d12s. I want to like poison spray. I wondered what people's experience with this cantrip are like. I would imagine Con is pretty typical save for many monsters. I had a really crazy and inefficient idea: I could take Thief of Five Fates with a Warlock and throw poison spray. It seems sinister and I like d12s.
As others have said, Poison is a common resistance (along with Fire) and quite a few do have immunity. It compensates for that with the d12 a bit, but also relies on a CON save, which CON is one of the highest ability scores overall in the game. Since creatures with high HP will likely have good CON scores, that d12 damage counts for even less.

Poison Spray: 2d12 damage, Con save to negate, poison resistance/immunity is common Shocking Grasp: 2d8 though with advantage (assuming a foe in metal armor), and foe can't take his reaction. My take is that weapon attacks are more reliable, in several ways. Hitting AC is easier at mid-levels than getting in a Con or Dex save.

Basic poison is of little help from the PHB, as it says. Poison, Basic. You can use the poison in this vial to coat one slashing or piercing weapon or up to three pieces of ammunition. Applying the poison takes an action. A creature hit by the poisoned weapon or ammunition must make a DC 10 Constitution saving throw or take 1d4 poison damage.
Should poison spray be a cone? Like the first image that comes up on Google when you search spray. Possibly 15', maybe scaling size up a bit as you level.
At the first four levels its a little more damaging than Poison Spray or Thorn Whip (depending on the Attribute), but those get another damage die at 5th Level and are therefore have not only more utility (pulling & range) but are also more damaging. The only advantage I see is that you can do Opportunity Attacks with the Shillelagh, but thats it.
