If it is the latter then I recommend you ENTER the data on the table (expand the log for additional values if needed) and then use the 'form' sheet as a 'report' sheet where you can either default to the 'last' line of the log or enter the Dispatch number and it can pull up all the other values from that log.
Request sign-off is basically automating the steps of the current user setting up columns manually, and recording approval status in those places. You are correct that ideally, it would record the update with the name of the approver, but that's not possible with Flow approvals yet. We hope we get to that capability in the future.
Would recommend you vote for it in order to push it up the agenda. Forms is a relatively young app so there is a lot of maturing still to do. Ctrl-[up][down] does NOT work; Ctrl-[home] does NOT work; Conclusion: problem is isolated to the configuration of one worksheet; Based on this, I researched more in terms of how the sheet was designed/developed (not by me). In my scenario, the problem stemmed from the worksheet being protected. Once I turned that off, navigation was back to normal!
