4.16.3* Height. The height of water closets shall be 17 in to 19 in (430 mm to 485 mm), measured to the top of the toilet seat (see Fig. 29(b)). Seats shall not be sprung to return to a lifted position. Appendix Note. and from the Appendix Note in question: Toilet seats 18 in (455 mm) high seem to be a reasonable compromise.
Both ADA (2010), in Section 604.8.1.2 and ANSI (2003), in Section 604.8.3, require that the toilet compartment doors provide maneuvering clearances. The 12" additional clearance, required if both a latch and a closer are present in a forward approach, push side situation is the same in both codes.
Based upon A117.1 604.3.3, shelves are allowed within the clear floor space for a toilet. The code section referred to by the OP would be 1003.3, which does not specify accessible routes, but circulation paths. The rear wall of a toilet room would not be an accessible route nor a circulation pathway. 1003.3 Protruding Objects
In residential dwelling units, the height of water closets shall be permitted to be 15 inches minimum and 19 inches maximum above the finish floor measured to the top of the seat. Interesting topic, consider that 40" is the approximate eye level height for those in WC's and it will also accommodate those who are height challenged. Consider also that if the mirror is tilted down towards you, it may allow for those who are follically impaired to see the thinning hair on top of their head, which most don't want to see ...
I have developers who want to use the pressure assist 0.8 GPM toilets in public housing. As you can see on this spec sheet, the flush actuator is a pushbutton on the top/center of the toilet. CBC 11B-604.6 and ADA 604.6 require that "flush controls shall be located on the open side of the water closet".
Toilet seats are shown at 12-14" AFF, and the lavatory rim/counter is shown at 30" AFF. The toilet compartments/stalls are all ambulatory compliant, however the wheelchair compliant compartment is not provided in this grouping, but rather in a separate bank of restrooms 10' away. The sinks in the training restroom are not ANSI compliant either.
A117.1 606.3 is silent on this matter. ADA and Chapter 11 of the IBC are also silent. For adults, the max height is 34" as we all know. 606.2 mentions clear floor space exceptions for children, but this isn't the info I'm after. I see articles online referencing a "29 inch max" height for children's but cannot find the source of this information.
2. Toilet rooms that are not accessible to the public, and have not more than one water closet. Accessories such as grab bars, towel bars, paper dispensers and soap dishes, provided on or within walls, shall be installed and sealed to protect structural elements from moisture.
