In contrast very little has ever been written about what constitutes a normal flow-volume loop and what this looks like has been primarily anecdotal. Interestingly, the ATS/ERS standard for spirometry includes an example of a “normalflow-volume loop but its source and what makes it normal is not explained.

The FEV 0.5 is considered more reproducible than the Peak Expiratory Flow and has been used to assess cough. Although normal values for the FEV 0.5 were included in the reference equations from the 1961 VA-Army and Kunudson’s 1976 spirometry study this measurement has rarely been used in adults but has instead found extensive use when ...

When this occurs it could be considered as being caused by two or more compartments of the respiratory tract being emptied with different time constants or being somehow gated differently at the start of exhalation. The one with the shortest time constant must be emptying later, than the initial one that shows the peak flow. By: Andrew Coates

Part of the reason for this may be that there is limited agreement about what a constitutes a normal VA/TLC ratio. Cotes et al suggest that the ratio decreases slightly with age and stated that the normal range is 0.9 to 1.0 at age 20 and 0.85 to 0.95 at age 60.

Recently I was trying to explain the effect of altitude on blood oxygenation to somebody with IPF. They had observed that their oxygen saturation was fairly normal at sea level but that they needed to use their supplemental O2 when they went up to 2000 feet and didn’t understand why such a low altitude made that much of a difference.
