I had to point out that the inspiratory flows on the loop in question came from the pre-FVC inspiration that started at FRC, not RV, and that we don’t emphasize maximal inspiratory flow at that point in the test, just maximal inspiratory volume and for these reasons the flow-volume loop was not a reliable indicator of inspiratory obstruction.
The flow-volume loop doesn’t look overly unusual although the expiratory flow doesn’t taper off to zero and the patient maintained a low expiratory flow for at least two-thirds of the vital capacity. Continue reading →

It was the first pulmonary function test that involved inspiratory and expiratory air flow in a significant way and for this reason it helped to set the stage both conceptually and technically for the FEV1, the FEV1/FVC ratio and our present understanding of obstructive lung diseases.

Recently while reviewing PFT reports I ran across a test from a patient who had been having spirometry, lung volume and DLCO tests performed at regular intervals for the last several years. Compared to the last several set of tests the most recent DLCO had decreased significantly while the FVC, FEV1 and TLC hadn’t changed.
One of the more recognizable flow-volume loop contours is the one associated with severe airway obstruction. Specifically, this type of loop shows an abrupt decrease in flow rate following the peak flow with a more gradual decrease in flow rates during the remainder of the exhalation.
