Pneumonia ranks 8th as a cause of death in the USA and is the most deadly of the infectious diseases. Poverty, age and access to vaccination and antibiotics are the main causes of global variation. In 2019, 2.5 million people died of ‘clinical’ pneumonia and pneumonia was the leading cause of death for children under 5.
Mycoplasma pneumoniae is a tiny parasitic bacterium in the class mollicutes (Latin for soft skin). They lack a cell wall, making them resistant to penicillins and invisible on Gram stain. M. pneumoniae causes upper and lower respiratory tract disease. It binds to ciliated epithelium preventing ciliary clearance.
Case Discussion. The dense lobar consolidation at admission shows some initial aeration at 1 week, but little in the way of radiological resolution, despite symptomatic improvement. At 5 weeks, there is almost complete resolution of consolidation.

Obscuration of the cardiac apex (i.e. silhouette sign) represents pathology in the lingula segment of the left upper lobe. In this setting airspace opacity with air bronchograms in the setting of the clinical presentation means this represents pneumonia.
