USP 800 (Hazardous Drugs—Handling in Healthcare Settings) is a guideline created by the United States Pharmacopeia Convention (USP), as one of their General Chapters through which the USP "sets quality standards for medicines, dietary supplements and food ingredients".
The goals were adapted from the JCAHO's National Patient Safety Goals. [1] Compliance with IPSG has been monitored in JCI-accredited hospitals since January 2006. [1] The JCI recommends targeted solution tools to help hospital to meet IPSG standards. [2]

The Agency for Healthcare Research and Quality (AHRQ) is the Federal authority for patient safety and quality of care and has been a leader in pediatric quality and safety. AHRQ has developed Pediatric Quality Indicators (PedQIs) with the goal to highlight areas of quality concern and to target areas for further analysis. [ 122 ]
The first edition of standards was released in 2006 and after that, the standards have been revised every 3 years. Currently the 5th edition of NABH standards, released in August 2020 is in use. The organization has to go for re-assessment after every 2 years. After every re-assessment, the renewal certificate is obtained by the hospital.
The Institute for Safe Medication Practices (ISMP) is an American 501(c)(3) organization focusing on the prevention of medication errors and promoting safe medication practices. [1] It is affiliated with ECRI .
It was designed for medication safety officers with the goal to provide an open forum for information sharing and collaboration. ASMSO was acquired by the Institute for Safe Medication Practices (ISMP) in 2013 and was renamed the Medication Safety Officers Society (MSOS). Membership in MSOS is currently free to all interested parties who register.
