U.S. Route 23 (US 23) is a 157.765-mile-long (253.898 km) United States Numbered Highway in the state of Kentucky.It travels from the Virginia state line near Jenkins to the Ohio state line west of South Shore via Jenkins, Pikeville, Coal Run Village, Prestonsburg, Paintsville, Louisa, Catlettsburg, Ashland, Russell, Flatwoods, Raceland, Wurtland, Greenup, and South Shore.
In Johnson County, the road intersects the southern terminus of KY 2381 before it passes to the northeast of Paintsville-Prestonsburg Combs Field, where it intersects KY 2559. The route heads north into residential areas and curves to the northeast. KY 321 continues north into dense forests, where it makes a turn to the west.
Prestonsburg is a small home rule-class city [6] in and the county seat of Floyd County, Kentucky, United States. [7] It is in the eastern part of the state in the valley of the Big Sandy River . The population was 3,255 at the time of the 2010 census , [ 8 ] down from 3,612 at the 2000 census.
It is 1.3 miles (2.1 km) from Prestonsburg, 13 miles (21 km) from Paintsville, 22 miles (35 km) from Salyersville, 23 miles (37 km) from Inez, 27 miles (43 km) from Pikeville, and 70 miles (110 km) from Ashland.
It is a 3-lane highway for 14 miles and then it merges with US 23 in Paintsville. US 460 East follows US-23 South through Prestonsburg and Pikeville . The route enters the southwestern part of Virginia .

US 460 leads northwest (downriver) 22 miles (35 km) to West Liberty and east 19 miles (31 km) to Paintsville. Kentucky Route 7 passes through downtown Salyersville with US 460 but leads southeast 36 miles (58 km) to Wayland. Kentucky Route 40 runs northeast out of Salyersville and also leads to Paintsville, reaching it in 18 miles (29 km).
