The special opens with Cookie Monster looking for directions to Sesame Street (and picking up cookies along the way). After a montage of various versions of the theme song, he arrives on Sesame Street with Joseph Gordon-Levitt who really wants to take a picture under the famous street sign but the trouble is it has gone missing.

In season 35 (2004), Prairie Dawn joined "Letter of the Day"; the segments in this incarnation were almost similar, with a foam letter used instead of an icing one. Segments produced in seasons 36 (2005) and 37 (2006) involved "The Letter of the Day Games": a game show introduced by an energetic off-camera announcer (voiced by Matt Vogel).
Prairie Dawn informs the viewers that the countdown continues on Sesame Street, while Elmo shows off how Japan celebrates the New Year, represented by his cousin Elmo-nosque, who shows how everyone makes cards for each other, helps to make rice cakes, and, during New Year's Day, plays home-made badminton.
A brief clip from Elmo's World appears in Sesame Street's 2002–2006 intro. In 2009, Elmo's World went on hiatus. It was replaced by Elmo: The Musical in 2012, and returned in 2017. The show is alternated by two more Elmo-centered segments, Elmo & Tango’s Mysterious Mysteries and Nature Explorers, which are both produced entirely with animation.
