The ovarian cycle consists of the follicular phase, ovulation, and the luteal phase; the uterine cycle consists of the menstrual, proliferative and secretory phases. Day one of the menstrual cycle is the first day of the period, which lasts for about five days. Around day fourteen, an egg is usually released from the ovary.
Figure 2. Follicular phase diagram of hormones and their origins. Follicle-stimulating hormone (FSH) is secreted by the anterior pituitary gland (Figure 2). FSH secretion begins to rise in the last few days of the previous menstrual cycle, [3] and is the highest and most important during the first week of the follicular phase [4] (Figure 1).
Follicle ‘selection’ is the process by which a single ‘dominant’ follicle is chosen from the recruited cohort or wave for preferential growth. It has generally been documented to occur once in the early- to mid- follicular phase of the menstrual cycle, leading to ovulation. [1]
In the early follicular phase, uterine contractions in the non-pregnant woman occur 1–2 times per minute and last 10–15 seconds with a low intensity of usually 30 mmHg or less. This sub-endometrial layer is rich in estrogen and progesterone receptors. [3] The frequency of contractions increases to 3–4 per minute towards ovulation.
Estradiol levels are minimal and according to most laboratories range from 20 to 80 pg/mL during the early to mid follicular phase (or the first week of the menstrual cycle, also known as menses). [ 63 ] [ 64 ] Levels of estradiol gradually increase during this time and through the mid to late follicular phase (or the second week of the ...
Calendar rules may set a standard number of days, specifying that (depending on a woman's past cycle lengths) the first three to six days of each menstrual cycle are considered infertile. [31] Or, a calendar rule may require calculation, for example holding that the length of the pre-ovulatory infertile phase is equal to the length of a woman's ...
