Normally you clean plugins before you start your playthrough, however cleaning can be done during it. There shouldn´t be any risk involved in doing so. Whenever steam updates your game, changes made to files can get reverted, .ini files tend to be safe with regards to updates, .esm shipped with the game are not.
In addition to jones177: There's a difference between load order for plugins and installation order for textures. The load order of your plugins is sorted by LOOT. The basic rule here is: What comes last in load order overwrites changes (to the same objects, settings and NPCs) made by mods that were loaded before.

1.4k. Posted December 13, 2017. Best practice is to only clean ITMs from your Update.esm and the three DLCs plus the UDRs of course. LOOT can only recognize that there are ITMs in that file and can not tell if those ITMs are necessary for that mod to function properly. Unless the mod author tells you otherwise you should clean UDRs from a file.
Downloading a mod with an esm plugin, or one flagged as such, MO2 will automatically place it towards the top of my LO when activated. When it does this it rearranges my LO. Create a folder in 'C:\Program Files (x86)\' called 'LOOT'. Extract its 'LOOT.v0.8.1.7z' In to the 'LOOT' folder you just created. Double click on LOOT and it should find Fallout 4. If it doesn't, go to the settings page and manually add the information in.
HarryD. I haven't touched Skyrim in a while and wanted to download some mods for it. Before I used Nexus Mod Manager (NMM), LOOT and TES5Edit and Wrye Bash to get my mods cleaned and installed. This worked well after I found a tutorial on how to do it. This is a new install of Windows 7 Pro and Skyrim so there aren't any programs affecting Skyrim.
The message does imply your INI files are broken. Have you tried deleting the INIs for the game and re-generating them? Indeed, I've deleted the fallout.ini files and falloutcustom.ini files and then launched the game to regenerate them.
Ghosting a plugin technically just means that the plugin gets renamed (foobar.esp.ghost) and thus can't get loaded by the game. At least older games like Oblivion apparently load plugins or at least waste some time on startup on disabled plugins so even a disabled plugin might have an effect on the game or at least negatively affect load times.
