Neighboring districts are (from the south clockwise): Tha Uthen, Si Songkhram, and Na Thom of Nakhon Phanom Province, and Bueng Khong Long of Bueng Kan province. To the east across the Mekong river is the Laotian province Khammouan.
The area was at first part of Tha Uthen district. On 1 April 1926, it was split off as the minor district ( king amphoe ) Akat Amnui (อากาศอำนวย), the present day Akat Amnui District in neighboring Sakon Nakhon was also originally controlled from Tha Uthen.
The name Nakhon Phanom, meaning 'city of mountains', was given to the city by King Rama I. There are no mountains within Nakhon Phanom itself, the limestone mountains being concentrated in the city of Thakhek in Laos on the other side of the Mekong River. Nakhon Phanom is more accurately described as a city where mountains can be seen.
