West of downtown Nashville, the three-mile (4.8 km) section between SR 155 (Briley Parkway/White Bridge Road) and US 70 (Charlotte Pike) was expanded to six lanes from February 1988 to December 1989. From April 1991 to December 1992, the 5.9-mile (9.5 km) section in Bellevue linking US 70 and US 70S was widened to six lanes.
SR 49 then goes through more hills and farmland before entering Charlotte and having an intersection and short concurrency with SR 48, just north of its intersection with SR 47. A previous segment about two miles (3.2 km) to the northeast ran from Charlotte Pike (US 70) to the former Cleeces Ferry location on the south bank of the Cumberland River; this was renamed Annex Avenue after the ferry closed. Old Hickory Boulevard south of US 70 and the section to the northeast renamed Annex Avenue never aligned with each other.
