Iliotibial band syndrome (ITBS) is the second most common knee injury, and is caused by inflammation located on the lateral aspect of the knee due to friction between the iliotibial band and the lateral epicondyle of the femur. [2] Pain is felt most commonly on the lateral aspect of the knee and is most intensive at 30 degrees of knee flexion. [2]
A varus force applied to the flexed knee when the foot is planted and the femur rotated externally result in a tear of the lateral meniscus. [10] Tears produce rough surfaces inside the knee, which cause catching, locking, buckling, pain, or a combination of these symptoms.
The POL, therefore, is not a stand-alone structure, but a thickening of the posteromedial joint capsule. It stabilizes internal rotation of the knee through all degrees of flexion but bears the most load when internally rotated in full extension. It also acts as a secondary external rotation stabilizer. [3] [4] [11]
Symptoms include the gradual, progressive onset of thigh or knee pain with a painful limp. Hip motion will be limited, particularly internal rotation. Running, and other strenuous activity on legs, will also cause the hips to abnormally move due to the condition and can potentially worsen the pain. Stretching is very limited.
Dial Test (posterolateral rotation test) - The dial test can be performed with a patient lying supine or prone. With the patient supine and the knees flexed 30° off the table, stabilize the thigh and externally rotate the foot. As the foot rotates, watch for external rotation of the tibial tubercle of the affected knee compared to the healthy ...
Age. The risk of most causes of joint pain increases with age. This may be due to increased wear and stress on joints over time and a higher likelihood of other underlying medical conditions ...
