Totally endoscopic coronary artery bypass surgery (TECAB) is an entirely endoscopic robotic surgery used to treat coronary heart disease, developed in the late 1990s. It is an advanced form of minimally invasive direct coronary artery bypass surgery , which allows bypass surgery to be conducted off-pump without opening the ribcage.
The Dor procedure is a medical technique used as part of heart surgery and originally introduced by the French cardiac surgeon Vincent Dor (b.1932). [1] It is also known as endoventricular circular patch plasty (EVCPP). In 1985, Dor introduced EVCPP as a viable method for restoring a dilated left ventricle (LV) to its normal, elliptical geometry.
This procedure makes heart surgery possible for patients who were previously considered too high risk for traditional surgery due to age or medical history. [ 5 ] [ 6 ] Patients referred for this procedure may have coronary artery disease (CAD); aortic, mitral or tricuspid valve diseases; or previous unsuccessful stenting.

With the heart in cardioplegic arrest , harvested arteries and veins are used to connect across problematic regions—a construction known as surgical anastomosis .
During the course of the docking process, the ligand and the protein adjust their conformation to achieve an overall "best-fit" and this kind of conformational adjustment resulting in the overall binding is referred to as "induced-fit". [5] Molecular docking research focuses on computationally simulating the molecular recognition process.
Minimally invasive direct coronary artery bypass (MIDCAB) is a surgical treatment for coronary heart disease that is a less invasive method of coronary artery bypass surgery (CABG). [1] MIDCAB gains surgical access to the heart with a smaller incision than other types of CABG.
This surgery had not been possible prior to 1975 because of difficulty with re-implanting coronary arteries which perfuse the actual heart muscle itself , and even after it was first performed the excellent results from the Mustard operation meant that it was a long time before the Jatene procedure took over.
