The southernmost island municipality of the Batanes island group, Sabtang comprises primarily Sabtang Island, as well as two nearby smaller and uninhabited islands: Ivuhos and Dequey. The municipality is known for its lighthouse and the old stone houses of the Ivatan villages of Chavayan and Savidug.

Sabtang Island, the nearest island to Batan is located about 4.5 kilometers (2.8 mi) southwest of the southern tip of the island. Itbayat, the largest island of the archipelago, is about 32 kilometers (20 mi) northwest of the central part of Batan.
The islands are almost equidistant to both Babuyan Island, Cagayan, which lies about 29 miles (47 km) to the southwest, and Sabtang Island, Batanes, located 28 miles (45 km) to the northwest. [note 1] The group is composed of seven small, sharp-peaked islets and rocks visible about 24 miles (39 km) in clear weather.
