He imagines the mannequins coming to life and tearing him apart, i.e. his guilty conscience finally gets the best of him and, in his eyes, literally tears him apart. He inflicts the wounds himself, however, I do think that it paid homage to the ending of the original, in which Frank stabs himself in the stomach but never dies.

Maniac was playing on IFC(I think), and I stayed up late and watched it. Scared the shit out of me. I had never seen anything that violent before at the time. For that reason, I have a special attachment to the original. Elijah's acting in the 2012 version is top notch, though. One of the best portrayals of a psychopath.

I watched Maniac (2012) last night with Elijah Wood, and was happily surprised that I really enjoyed it! I don't normally enjoy newer slasher films, but found this one to be very intriguing, not be completely predictable and had the right amount of suspense and gore.
