    You must submit a Compass Authorisation Form (CAF) so they can approve the change. This form must be received within 7 days of the amendment. You must send your CAF forms to dentalcaf@nhsbsa.nhs.uk if your Integrated Care Board (ICB) is based in: London; Cheshire and Merseyside; East of England
