It can be found in various products which include chewing gums and lozenges. Xylitol has been found to reduce mutans streptococci in plaque and saliva and reduce the binding of these to the acquired enamel pellicle. [10] This in turn leads to less adherent plaque and a decrease in acid production. [10]
One of the physical changes can be the colour of teeth. Dental erosion can lead to two major tooth colour change – the first being a change of colour that usually happens on the cutting edge of the central incisors. This causes the cutting edge of the tooth to become transparent. [17] A second sign is a yellowish tint on the eroded tooth.
This can associated with the consumption of acidic foods and liquids or regurgitation of stomach acid, a process known as dental erosion. An increase in acidity at the tooth surface can induce demineralization and softening, therefore leaving the tooth structure susceptible to abrasive factors such as tooth brushing. [ 18 ]
These forms of tooth wear can further lead to a condition known as abfraction, [3] where by tooth tissue is 'fractured' due to stress lesions caused by extrinsic forces on the enamel. Tooth wear is a complex, multi-factorial problem and there is often difficulty identifying a single causative factor. [ 3 ]
Nonetheless, enamel can sometimes be removed before there is any decay present. The most popular example is the dental sealant. In the past, the process of placing dental sealants involved removing enamel in the deep fissures and grooves of a tooth, followed by replacing it with a restorative material. [38]
[4] [12] These lead to a concentration of stress and flexion at the area where the enamel and cementum meet (CEJ). [3] [5] This theoretical stress concentration [13] and flexion over time causes the bonds in the enamel of the tooth to break down and either fracture or be worn away from other stressors such as erosion or abrasion.
