Cervical spinal nerve 4, also called C4, is a spinal nerve of the cervical segment. It originates from the spinal cord above the 4th cervical vertebra (C4). It contributes nerve fibers to the phrenic nerve , the motor nerve to the thoracoabdominal diaphragm .
Spinal nerve Typical spinal nerve location. Each spinal nerve is a mixed nerve, formed from the combination of nerve root fibers from its dorsal and ventral roots. The dorsal root is the afferent sensory root and carries sensory information to the brain. The ventral root is the efferent motor root and carries motor information from the brain.

The cervical spine section contains seven vertebrae, C-1 through C-7, and eight nerve pairs, C-1 through C-8.
The first 4 cervical spinal nerves, C1 through C4, split and recombine to produce a variety of nerves that serve the neck and back of head. Spinal nerve C1 is called the suboccipital nerve, which provides motor innervation to muscles at the base of the skull.
