Apollo 13 (April 11–17, 1970) was the seventh crewed mission in the Apollo space program and would have been the third Moon landing.The craft was launched from Kennedy Space Center on April 11, 1970, but the landing was aborted after an oxygen tank in the service module (SM) exploded two days into the mission, disabling its electrical and life-support system.
In Chapter 13 of Apollo Expeditions to the Moon (1975), Jim Lovell recalls the event: "Jack Swigert saw a warning light that accompanied the bang, and said, 'Houston, we've had a problem here.' I came on and told the ground that it was a main B bus undervolt. The time was 21:08 hours on April 13." [5]

Apollo astronaut Thomas Kenneth Mattingly II, known for helping the crew of Apollo 13 safely return to Earth after an explosion doomed their lunar mission, has died at the age of 87, NASA announced.
The crew were exposed to the toxic fumes from 24,000 ft (7.3 km) down to landing. About an hour after landing the crew developed chemical-induced pneumonia and their lungs had edema. They experienced shortness of breath and were hospitalized in Hawaii. The crew spent five days in the hospital, followed by a week of observation in semi-isolation.
In 1970, Mattingly was supposed to have joined the crew of Apollo 13, piloting the command module. But he was removed from the mission a few days before launch after being exposed to German measles.
Given that the survival of the Apollo 13 crew was largely due to the excellent system in place to handle problems, this should be explained. From a quick skim, the Training and preparation section doesn't seem to cover how Mission Control also prepared for the mission.
