Ocular neuropathic pain, also called corneal neuralgia, is a spectrum of disorders of ocular pain which are caused by damage or disease affecting the nerves.Ocular neuropathic pain is frequently associated with damaged or dysfunctional corneal nerves, [1] but the condition can also be caused by peripheral or centralized sensitization. [2]
It results in congestion of the eyeball, often eye-watering, redness and swelling, itching and burning, and a general feeling of irritation under the eyelids. Ophthalmia can have different causes, such as infection from bacteria, viruses, fungi, or may result from a physical trauma to the eye, chemical irritation, and allergies.
Gastric bypass surgery may also cause a vitamin B 12 deficiency from poor absorption. Patients who have nutritional optic neuropathy may notice that colors are not as vivid or bright as before and that the color red is washed out. This normally occurs in both eyes at the same time and is not associated with any eye pain.
The affected eye may have increased tears or be "stuck shut" in the morning. [1] Swelling of the sclera may also occur. [1] Itching is more common in cases due to allergies. [3] Conjunctivitis can affect one or both eyes. [1] The most common infectious causes in adults are viral, whereas in children bacterial causes predominate.
Oculomotor nerve palsy or oculomotor neuropathy [1] is an eye condition resulting from damage to the third cranial nerve or a branch thereof. As the name suggests, the oculomotor nerve supplies the majority of the muscles controlling eye movements (four of the six extraocular muscles, excluding only the lateral rectus and superior oblique).
There are many diseases known to cause ocular or visual changes. Diabetes , for example, is the leading cause of new cases of blindness in those aged 20–74, with ocular manifestations such as diabetic retinopathy and macular edema affecting up to 80% of those who have had the disease for 15 years or more.
However, if the eyes are asymmetrically affected, i.e. one eye's optic nerve is more damaged than the other, it will produce an important sign called an afferent pupillary defect. [citation needed] Defective light perception in one eye causes an asymmetrical pupillary constriction reflex called the afferent pupillary defect (APD). [citation needed]
The most obvious presentation of ocular (eye) injuries is redness and pain of the affected eyes. This is not, however, universally true, as tiny metallic projectiles may cause neither symptom. Tiny metallic projectiles should be suspected when a patient reports metal on metal contact, such as with hammering a metal surface.
