One breast deals with elasticity while the other doesn't. 19mm fits both at nipple base, though it hurts on both to use. The elasticity one I use a 21mm as it expands to fit the flange no matter the size. On the other breast I am figuring out what size between 21-24mm is best.

Ive tandem nursed two men on two separate occasions. If you enjoy nursing one man, you can only imagine how much better it feels to nurse two. Yes my late wife was tandem suckled by myself and a good friend on a few occasions. She was lactating fully from post pregnancy and had high milk production.
Definitely would have to have rules and probably suggest being fully clothed and just use ur nursing bra to expose the one breast.
