HIV disease–related drug reaction is an adverse drug reaction caused by drugs used for the treatment of HIV/AIDS. Image of common antiretroviral drugs to treat HIV infection [ 1 ] Drug reactions are a major cause of mortality and morbidity around the world.
The suppression of CD4 T cells by HIV (or by immunosuppressive drugs) causes a decrease in the body's normal response to certain infections. Not only does this make it more difficult to fight the infection, it may mean that a level of infection that would normally produce symptoms is instead undetected (subclinical infection).
Figure 1. Early Symptoms of HIV. The stages of HIV infection are acute infection (also known as primary infection), latency, and AIDS.Acute infection lasts for several weeks and may include symptoms such as fever, swollen lymph nodes, inflammation of the throat, rash, muscle pain, malaise, and mouth and esophageal sores.
HIV seeks out and destroys CCR5 expressing CD4 + cells during acute infection. A vigorous immune response eventually controls the infection and initiates the clinically latent phase. However, CD4 + T cells in mucosal tissues remain depleted throughout the infection, although enough remain to initially ward off life-threatening infections.
Over time, they cause acquired immunodeficiency syndrome (AIDS), [1] [2] a condition in which progressive failure of the immune system allows life-threatening opportunistic infections and cancers to thrive. [3] Without treatment, the average survival time after infection with HIV is estimated to be 9 to 11 years, depending on the HIV subtype. [4]
Acquired immunodeficiency syndrome (AIDS) is defined as an HIV infection with either a CD4 + T cell count below 200 cells per μL or the occurrence of specific diseases associated with HIV infection. [32] In the absence of specific treatment, around half of people infected with HIV develop AIDS within ten years. [32]
Zalcitabine was the third antiretroviral to be approved by the Food and Drug Administration (FDA) for the treatment of HIV/AIDS. It is used as part of a combination regimen . Zalcitabine appears less potent than some other nucleoside RTIs, has an inconvenient three-times daily frequency and is associated with serious adverse events.
HIV-associated nephropathy (HIVAN) refers to kidney disease developing in association with infection by human immunodeficiency virus, the virus that causes AIDS. The most common, or "classical", type of HIV-associated nephropathy is a collapsing focal segmental glomerulosclerosis (FSGS), though other forms of kidney disease may also occur. [ 1 ]
