A social determinants lens is crucial when addressing chronic diseases in Northern Ontario (Marmot, 2005). The determinants of health (listed in Appendix A) are defined as the diverse set of factors that affect health and pertain to the conditions in which people are born, grow, live, work, and age (WHO, 2012b).
The concept of the "health field," as distinct from medical care, emerged from the Lalonde report from Canada. The report identified three interdependent fields as key determinants of an individual's health. These are: [14] Biomedical: all aspects of health, physical and mental, developed within the human body as influenced by genetic make-up.
PHM helped to put the Social Determinants of Health on the global agenda through continued engagement with the World Health Organization. [9] In response Prof. Fran Baum (Australia), at that time Chair of the PHM Steering Council, was appointed as a commissioner on the WHO Commission on Social Determinants of Health (CSDH).
